Orlistat is a pancreatic lipase inhibitor, meaning it stops the enzyme responsible for breaking down fat for absorption in the intestine. As far as weight loss aids go it is pretty benign, but it can have some nasty side effects (steatorrhea – fat in your feces, if you eat too many fatty foods).

I’m not sure you would benefit from this drug IF you are on a lower-fat diet due to your cholecystectomy – I know most people return to a normal diet after the liver adapts, but some have difficulty tolerating fatty foods. If you are on a lower-fat diet, there will be less fat to block, and so Orlistat won’t do anything to help you.

Of course, this all begs the question of whether you are using this as a weight loss AID, or as a magic diet pill. If the former, you may see benefits (although I’m somewhat skeptical – see above). If the latter, then you need to speak to your physician to get a referral to an RD (registered dietitian) who can work with you on altering your eating habits to promote weight loss.

Orlistat is contraindicated in cholestasis and chronic malabsorption syndrome. If you have had a cholecystectomy taking Orlistat may potentially be harmful (increased malabsorption and potential for diarrhea). I would consult a physician before taking this medication.

With that said, why would you want to take Orlistat? Basically, the way this medication works is by binding to the excess fats in your food and then immediately excreting them out (preventing lipase activity). If you have a low fat diet already, this medication wouldn’t really add any benefit. In addition, the people that have used this medication have to constantly watch what they eat, 100% of the time, because if they don’t, they have only a small amount of time to get to the bathroom before they crap their pants. So, sure it’s effective, but only because you live in fear of complete public humiliation the entire time you’re on the medication.
